(only on working days). The forms @ 300/= are available at the school counter
between 8:30 a.m. and 11:30 a.m.

01.04.2013)
A duly filled-in form, together with a photocopy of the
01 Date of Birth Certificate obtained from the Municipal
Corporation.
02.Medical Certificate of the child (for children with special
needs)
03. Pass out certificate of
Father & Mother (School, Graduation, Post Graduation etc)
04. Address proof of
residence.
05. Cast Certificate.
06. Proof of sibling (Photo
copy of fee receipt) whether applicable.
07. Proof for Alumini.
(whether applicable)
08. A passport size
photograph of the child.
Last date of submission of form : 20.12.2012
Note – Date for verification of
certificate will be notified on 24.12.2012
